Coaching enquiry form builder
Tick the fields you will actually use. Get a clean form for the front desk — printable, or copy it as text. No signup.

Add only if you will use it
6 fields selected. Good length for a walk-in. A parent will finish this standing at the desk.
A printed form still has to be typed into something before anyone can follow it up. The QR version skips that step — the parent fills it on their own phone and the enquiry lands in your CRM already assigned.

Questions
What fields should a coaching institute enquiry form have?
Student name, parent name, mobile number, class, the course they are asking about, and how they heard about you. Six fields. The mobile number is the only one you genuinely cannot lose, and 'how did you hear about us' is the one that tells you which of your marketing actually works.
How many fields is too many?
Past about nine, completion drops sharply — particularly at a counter, where a parent is standing and often has a child with them. Ask for what you will act on this week; the rest can be captured during the follow-up call.
Is this enquiry form builder free?
Yes. It runs entirely in your browser, needs no signup, and nothing you type is uploaded or stored.
Should we use a paper form or a digital one?
Paper works at the counter, but somebody still has to type it in before anyone can follow it up — and that gap is where enquiries get lost during admission season. A digital form that writes straight into your CRM removes the retyping step entirely.
Skip the retyping
A printed form still has to be entered into something before anyone can follow it up. With QR registration the parent fills it on their own phone and the enquiry arrives in your CRM already assigned to a counsellor.
